New Patient Registration Process

Click the “Book an Appointment” button on our website.

Click “Book Now.”

Select the clinic location of your choice. (Currently, only the Kingsway location is accepting new patients.)

Select “New Patient Registration.”

Register yourself and complete the intake form.

After submitting the intake form, the system will prompt you to book a meet-and-greet appointment with the available doctors from the list.

Select the doctor of your choice and choose a date and time.

If you are registering more than one patient, you must submit a separate registration and intake form for each patient and book separate appointments for each.

Your MSP coverage must be valid. If your MSP is invalid or missing, there is a $100 charge per visit.

All mandatory sections of the intake form must be completed. Incomplete applications will not be processed.

Dr. Rogelio maintains a limited patient panel to ensure he can provide comprehensive, high-quality care for each patient. He also accepts sports medicine and musculoskeletal referrals.
Dr. Rogelio’s intake form remains available on our website; however, submission of the form does not guarantee acceptance as a patient. Due to the limited number of openings, not all applicants will be accepted.
We encourage you to also submit a general intake form to VyCare Medical Clinic (link given above). This allows our team to consider booking you with another physician at the clinic who may be accepting new patients sooner.
If you are seeking a family physician, you may also wish to explore other resources such as the provincial patient attachment programs or the Find a Doctor registry.
