New Patient Registration Process

• Click the “Book an Appointment” button on our website.​

• Click “Book Now.”​

​

• Select the clinic location of your choice 

​

• Select “New Patient Registration.”​

​

• Register yourself and complete the intake form.​

​

•  System will prompt you to book a meet-and-greet appointment with the available doctors.

​

• Select the location and doctor of your choice and choose a date and time.​

 

• ​You must submit a separate form for each patient and book separate appointments for each patient.​​

​

• Your MSP coverage must be valid. If your MSP is invalid or missing, there is a $100 charge per visit.​

​

• All mandatory sections of the intake form must be completed. Incomplete applications will not be processed.

​
