A female family doctor with a focus on (women wellness) for females aged 20-45 is accepting new patients, their kids and partner on a first come-first served basis.

Please fill out the intake form below and email to info@vycaremedical.ca

1. Only complete intake forms will be accepted.

2. Fill out a separate intake form for each patient. Do not submit one form for the entire family.

3. Our office will email you to confirm the receipt of your form and invite you to call us and book a meet and greet appointment. Do not call our office to follow up.

4. Please note that the meet and greet is to determine if the physician can meet your healthcare needs and become your optimal care provider. Therefore, it is not guaranteed that you will be attached to the physician. Patient attachment is at the physician's discretion.
